About the Iberian outage on April 28...
A final report is yet to be released so more information will be available. Here is an update on the investigation as of May 15.
I have not heard that any equipment was damaged, so the safety shutoff feature of the grid apparently worked. That is small consolation to those killed, injured, or had their lives interrupted in some way during this disaster.
The first outage was at a substation in Granada. Attempts to shed load did not stop the spread of outages through the grid.
Before the outages started at 12:32 pm there were oscillations on the system.

I believe the path of the investigation is to track down the source of the oscillations. They are looking inside the Iberian grid and in other connected European grids.
